POG Incident Database
The POG Incident Database is hosted on another server. It is accessible via the link at the bottom of this section.
The POG Incident Database is used to record all near miss and incident events of pipelines designed and constructed in accordance with the scope of AS2885.1 (and superseded Standards) and operated and maintained in accordance with AS2885.3. The Incident Database is used to determine Australian and New Zealand incident and near miss frequency rates that can be used in qualitative and quantitative risk assessments. It is also used to determine if current Australian Standard requirements are appropriate for the frequency of the different types of incidents.
The definitions of reportable events are:
Incidents:
- Loss of containment (excluding minor flange leaks).
- Damage to pipe or coating caused by impact (mechanical or any other means).
- Repairs to pipe involving mechanical reinforcement (which if not undertaken would require an MAOP reduction) or cut-out and replacement.
- MAOP reduction due to a pipe defect.
Near misses:
- Unauthorized third-party activities which do not damage the pipeline with land Disturbance deeper than 300 mm within the easement, or within 3m of the pipe if there is no easement.
- An Erosion or Earth Movement Near Miss Event is where the pipeline has been displaced from the original alignment or requires restoration of depth of cover greater than 300mm (including the pipeline being exposed, surface erosion and waterway erosion) or where there is erosion or landslip/landslide/slump needing repair to prevent the pipeline being under the threat of displacement or excess strain. The pipeline has not undergone any coating or pipeline damage.
- Unauthorised seismic activity and use of explosives in the vicinity of the pipeline.
